一、中國醫藥大學為辦理實驗室安全衛生教育訓練研習(以下簡稱本活動)取得您的個人資料，目的在蒐集、處理及利用基於執行業務需求事由，您所提供或未來基於各種事由所提供的個人資料將受到中華民國「個人資料保護法」及相關法令之規範。
China Medical University, may need to collect, process, and use your personal information, for the purpose of executing affairs related to Laboratory Safety and Health Training Course.. The information provided by you are protected under Article 8 of Personal Information Protection Act.
二、本活動因執行業務所蒐集到您的個人資料如報名系統所示，包含所屬機構及單位、姓名、職稱、聯絡方式、E-MAIL、學號或入口網站帳號...等。
With regard to the Laboratory Safety and Health Training Course, the following categories of your personal information that may be collected include: organization and department, name, title, contact number, student ID, school portal account and etc.
三、您所填寫之資料供本次實驗室安全衛生教育訓練研習使用，使用者所填寫之個人資料，本單位將予以保護，防止被誤用、未經授權的存取或公開。
The personal information from you is only for the Laboratory Safety and Health Training Course. The department will protect and prevent your information from misuse or unauthorized access and publish.
四、您可依中華民國「個人資料保護法」，以書面主張下列權利：
(一)請求查詢或閱覽
(二)請求製給複製本
(三)請求補充或更正
(四)請求停止蒐集、處理或利用
(五)請求刪除。
According to the regulations stipulated in Article 3 of Personal Information Protection Act, you are entitled to exercise the following rights with regard to personal information you provided:
(1) Any inquiry and request for the review of your personal information;
(2) Any request for duplications of your personal information
(3) Any request to add on or correct your personal information
(4) Any request to discontinue the collection, processing or use of your personal information;
(5) Any request to delete your personal information
五、您若不同意簽署本告知暨同意書，將無法為您完成實驗室安全衛生教育訓練研習報名, 亦可能無法維護您的權益。
If you refuse to provide personal information, the department will not be able to complete the course registration and may not be able to protect your rights.
六、您瞭解此一同意書符合個人資料保護法及相關法規之要求，具有書面同意本網站蒐集、處理及利用您的個人資料之效果。
You understand that this agreement complies with the requirements of the Personal Information Protection Act and relevant regulations, which has the effect of agreeing for the website to collect, process and use your personal data.
